- Public healthcare in Catalonia is the CatSalut system. Access is through the tarjeta sanitaria individual (TSI) and the CAP, the primary care centre; other services are reached by referral from there [1][4][5].
- The entitlement is recognised by the INSS for insured people and their family members; other residents receive care under the conditions of the law and CatSalut’s rules [1][3].
- For the TSI you need empadronamiento in a Catalan municipality and an identity document: a registration certificate no older than three months [4].
- Without entitlement to public care, a convenio especial is possible: €60 a month under 65, €157 from 65; you need one year of actual residence in Spain and empadronamiento [2].
- Private insurance is mandatory for some visas: the student visa requires a policy from an insurer authorised to operate in Spain, with cover comparable to the public system’s basic package [6].
How does public healthcare in Catalonia differ from private?
The public system in Catalonia is run by CatSalut, and care is provided by primary care centres (CAP) and hospitals in the public network. The TSI confirms your entitlement. Private healthcare is a contract with an insurer or clinic in which you pay for access and services. They differ in how you get in, the right to the service and who pays.
| Question | Public (CatSalut) | Private |
|---|---|---|
| Who determines entitlement | The law: insured people and family members, residents under the conditions of the law [1][3] | The contract with the insurer or clinic |
| What you need to get in | Empadronamiento, identity document, TSI [4] | A signed contract or payment for the service |
| First contact | CAP, without referral; other services by referral from there [5] | Set by the contract |
| Who pays | Public funds, if entitlement is recognised [3] | You or your employer |
Who is entitled to public healthcare in Catalonia?
The law on the cohesion and quality of the national health system (Ley 16/2003, article 3) recognises the right to health protection for Spanish citizens and for foreign nationals who have established their residence in Spain. To receive care funded by the state, a foreign national must be legally and habitually resident and must not be required to prove cover by other means [3].
CatSalut explains it this way: all residents of Catalonia have guaranteed healthcare (Llei 9/2017 on universal access, page dated 22.12.2025), and care funded by CatSalut requires the status of insured person or beneficiary, which is recognised by the Instituto Nacional de la Seguridad Social (INSS). Those without that status receive care through separate procedures [1].
Ley 16/2003 adds two routes: someone without entitlement under the general rule can sign a convenio especial and pay a contribution (art. 3.3), and foreign nationals without legal residence are entitled to state-funded care under the conditions of art. 3 ter, if there is no other mandatory cover [3]. For people in work this is usually linked to registration with Seguridad Social: the employer files the alta before work starts, more in the article alta and RETA [7].
Foreign nationals outside insured status can get access if they meet the conditions: three months of continuous empadronamiento, household income below the basic threshold, no other public cover [2]. This CatSalut page is dated 11.01.2017 and refers to rules from 2012: check the conditions again at the CAP.
What do you need to start using public healthcare?
- Register your empadronamiento in a Catalan municipality: without it, the TSI is not issued [4]. How to do it: empadronamiento in Barcelona.
- Apply for the TSI online or at the CAP where you live, with an identity document (DNI, NIE or passport) and a registration certificate no older than three months [4].
- Wait for the card: CatSalut sends it by post free of charge, and in response to an online application it emails a provisional confirmation with your personal CIP code [4].
- Book an appointment with a doctor at your CAP: how to register with a family doctor. More about the card: tarjeta sanitaria in Catalonia.
When do you need private insurance?
Private insurance is needed in three cases. First, if an immigration procedure requires health cover: for example, for a student visa, regulation 1155/2024 requires a policy from an insurer authorised to operate in Spain, for the whole stay, with services comparable to the public system’s common basic package [6]. Second, if you do not yet have entitlement to public care or are not sure you meet the conditions. Third, if you choose additional services yourself.
For a student who works under an authorisation, the insurance requirement is met by registration with Seguridad Social [6]. Requirements for other grounds differ: health insurance for a residence permit and insurance for the DNV: UGE criteria. We do not name or recommend insurers or clinics.
Can you get public care without entitlement through the Seguridad Social?
Yes, if the conditions are met. A convenio especial under Real Decreto 576/2013 gives access to people who are not insured and have no public cover elsewhere. CatSalut states a contribution of €60 a month under 65 and €157 from 65; the applicant proves actual residence in Spain for at least one continuous year and empadronamiento in Catalonia at the time of applying. The decision is made by the head of the health region within 30 days, and care applies from the signing of the agreement [2].
CatSalut describes special cases separately (minors without authorisation, pregnant women, people seeking international protection): the application is made at the CAP [2].
What depends on your case, and what do we not know?
Who you are under the law (an insured person, a family member, a resident without Seguridad Social registration, an applicant without a permit) depends on your status, your contract and the type of residence permit. We do not compare waiting times for specialists, quality of care or prices of private plans: we did not find comparable official data for such a comparison. Start from the service you need and what your status gives you, not from impressions. The cover required for your residence ground is described in the route card for no lucrativa and in the overview residence permit in Barcelona.
